Parent/Guardian Agreement Forms
Parents/Guardians of participating students are required to complete the Parent/Guardian Agreement Forms (Participation Permission; Medical Treatment Authorization; and Risk Sharing/Indemnification Agreement.)

Cancellation Policy
To cancel your registration, please submit your request in writing by February 20, 2012, to receive a refund. Cancellations should be faxed to TABS at 828-258-6428.

A $100 processing fee will be applied. No refunds are made for cancellations received after February 20, 2012; however TABS will transfer your registration to another staff member from your school. Weather-related refunds will not be granted.
